A copy of the veteran's military medical records
Required. VA medical treatment and/or civilian medical records may be uploaded alongside them to support military-related conditions.
- VA medical records (if applicable)
- Includes any treatment received through the VA healthcare system.
- Fitness reports or medical board proceedings
- Documents related to medical discharges or assessments of fitness to serve.

DD214, plus relevant service information
A DD214 is required. If the veteran is submitting a Benefits Delivery at Discharge (BDD) claim within 180 days from official discharge and does not have a DD214, they will use a Statement of Service document signed by their Commander or Personnel Officer.
Not required but recommended is to have relevant military service information: e.g. narratives of combat awards (Purple Heart, Bronze Star w/ V, etc.); airborne log; school certificates; etc. Expanded examples:
- Awards and decorations
- Verifies service in specific operations or exposure to unique conditions (e.g. a Combat Action Ribbon, Combat Infantryman Badge, Combat Action Badge for combat-related PTSD claims).
- Buddy statements
- Written statements from fellow service members to verify incidents or conditions experienced during their service.
- Unit records or logs
- Supports claims for injuries or stressors tied to specific unit activities or missions.
- Exposure records
- Documentation of exposure to toxic substances, environmental hazards, or traumatic events (e.g., chemical exposure logs or blast exposure records).
- Deployment records
- Verify locations, combat zones, or exposure to specific hazards (e.g. Agent Orange, burn pit exposure, etc.).
- Incident reports or line of duty determinations
- Details specific events that led to an injury or condition, such as accidents, combat injuries, or training incidents.
Veterans you serve missing their medical records?
If a veteran your organization is assisting doesn't have a digital copy of their military medical records, VetXpert Solution has a preferred partnership with a VA-accredited attorney who can help obtain them.
